A copy of the MSHA 5000.23 form will be required to be provided for each individual worker. All workers have to complete the on-line convergence training (see attached instructions) prior to arrival, and must print and turn in the completed form upon first arrival. The Joppa Plant has a clean shaven policy with facial hair for all workers that will be working on plant property. For items being sold at locations other than the Joppa Plant, a certificate of insurance will suffice ($2,000,000 General Liability - see details below). Last day for removal is Friday, March 29, 2019.

RE: (auction site address) AMOUNTS: Minimum Comprehensive General Liability Coverage of USD $2,000,000 on an Occurrence Basis. In case the work or service involves transportation, storage, handling, or use of commercial explosives and blasting agents we require a USD $5,000,000 limit and the certificate of insurance must state that explosives and blasting are covered. These limits may be evidenced through a Commercial General Liability policy and an Umbrella policy if necessary to reach the required limit. Complete insurance requirements are listed in Appendix 2 (pages 13 – 14) of the Contractor Safety Management document.
